Flu Shot Clinic

3:30 pm - 6:00 pm

Monroe County Health Department will be conducting a Kids Flu Shot Clinic at the Tomah High School in the Math Lab near the Large Gym on October 17th from 3:30-6PM. Please see the Vaccine form below for the printable PDF form to have it completed before your child attends the clinic.
